Canton Mancan is assisting a local company in hiring a Social Worker for a full-time position in Canton, OH. The ideal candidate will hold a Bachelor’s Degree focused on health, education, or welfare of children, possess three years of direct client service experience, including one year of administrative experience in a health or human service program, and hold a valid Ohio driver's license with access to a car and car insurance. The candidate should demonstrate the ability to interact positively with others and be willing to align with the mission and goals of the Margaret B. Shipley Child Health Clinic, Inc.
RESPONSIBILITIES:
The Care Coordinator /EHS Supervisor is responsible for coordinating care at the Clinic and supervising the day-to-day activities of home visiting staff. Serving as the liaison with other agencies and team leaders, the Care Coordinator/EHS Supervisor responds to the clinic pediatrician and reports directly to the Executive Director.
FUNCTIONS:
Completing admissions and assessing social needs.
Providing patients/families with age-appropriate information and/or literature.
Offering direct service and making appropriate referrals for community services.
Communicating with physicians and staff, and providing written documentation regarding treatment, progress, and community service involvement.
Maintaining current knowledge of EHS guidelines, community resources, and Clinic Policies.
Coordinating the Clinic’s Reach Out and Read program.
Providing guidance and supervision of day-to-day activities of EHS home visiting staff.
Monitoring EHS staff development, competency, quality, and productivity.
Overseeing data collection, reports, and home visiting staff needs.
Assisting in the hiring, training, supervision, and evaluation of EHS home visiting staff.
Offering guidance and assistance for Health Services and Nursing staff.
Performing other tasks as requested by the Executive Director or physician.
Participating in continuing education.
